Lean on `Logger` wrapping in `ChannelMonitor` rather than in channel

Now that `ChannelMonitor` is careful about wrapping `Logger`s at
the edge, there's no need to use `WithChannelMonitor` in a few
cases in `channel.rs` and one in `channelmanager.rs`.
